Insert a long, thick lemon zest running all over the inside of the glass.

Optionally pour the alcohol of your choice, then complete with the ginger ale.
History
The Horse's Neck cocktail first appeared in 1895 in "Modern American drinks", a book written by the American bartender George Kappeler who is certainly the originator of this recipe.
Horse's neck means "horse's neck", and it is originally a non-alcoholic drink, simply composed of ginger ale and a long lemon zest running throughout the interior of the glass. This specific decoration already existed and is therefore not an invention of George Kappeler.
* Originally alcohol-free, Horse's Neck was however released in the decades that followed with all kinds of alcohol: gin, brandy, whiskey or rum. You are free to use the brandy you prefer, or not to include any!
